Job Description
- Competitive Pay of $24.00 per hour
- Schedule: Mondays – Fridays 8:00am to 5pm
- Location: Houston, Texas

- Evaluate and supervise all Agency care team staff for conformity with professional standards of home care and in accordance with state health statues and home care licensure requirements.
- Ensures the quality of patient care and service by using specialized knowledge and judgment to assure patient care standards are in compliance with applicable State Practice acts and Agency policies.
- Performs or supervises case management activities for individual patient’s, including but not limited to assessments, teaching and development, and updating the care plan or task sheets.
- Coordinates arrangements with other healthcare and equipment providers to assure continuity of care and adherence to the patient plan of care.
- Provides specialized training for care team members per client’s plan of care and professional home care standards.
- Helps improve the knowledge base and effectiveness of agency care team staff by teaching training sessions.
- Participates in QAPI meetings, reviewing client surveys, client complaints, infection log, incident reports and making recommendations to the management team on improving the quality of care.
- Three (3) to five (5) years of Care Manager experience within a Homecare organization or other healthcare setting.
- Must have completed High School Diploma or GED.
- $24.00/hr.
